ATLANTIS Health Plan
Atlantis POS/HMO Plans Atlantis plans are available to employer groups as
small as 1 in the New York 5-Boroughs region. Atlantis POS plans offer both
in and out-of-network benefits. Health care services are available through
the Atlantis provider network consisting of 28,000+ providers and 43
hospitals located within their service region. An individually-selected
Primary Care Physician will be responsible for each family member's care and
may refer subscribers to out-of-network physicians or facilities. Atlantis
HMO plan services are accessed in the same manner, but are limited to
in-network physicians and facilities only. |
